How To Submit A New Affiliation Agreement and Clinical Site Location Request in Salesforce

Instead of using  Microsoft Forms to request a new Affiliation Agreement, these requests will now be submitted in Salesforce (beginning September 2025). Once the requests are submitted, the Agreement and Compliance Manager will review and submit for approval. Overview

Instead of using  Microsoft Forms to request a new Affiliation Agreement, these requests will now be submitted in Salesforce (beginning September 2025). Once the requests are submitted, the Agreement and Compliance Manager will review and submit for approval. This article explains how to check if the Affiliation Agreement already exists and how to create a new one.

In addition to an new Affiliation Agreement, a new Clinical Site Location will also need to be tied to the agreement. This article also explains how to do this. 

Search Salesforce to see if Affiliation Agreement already Exists

  1. Log in to Salesforce: https://ouhcom.my.salesforce.com/
  2. Make sure you are in the 'Preceptor' application via the waffle

 

3. In the Search Bar at the top of the page - search for the name of your Affiliation Agreement. 

 

4. When the search results appear - you're looking for an Affiliation Agreement Record. You'll likely have to scroll down a bit to find that area. 

5. If your new Affiliation Agreement does not exist, create a new request. 

6. If your Affiliation Agreement exists, but is inactive - please email hcom.agreements@ohio.edu, notifying them you'd like to reactivate it. Please include any pertinant information. 

Requesting a New Affiliation Agreement

  1. Return to the 'Home' tab and click the green 'Affiliation Agreements' tile. 

 

2. On the page that loads, click the 'New' button in the right hand corner.

 

 

3. Enter data in the 'Intake' section of the page. It will be the same data points collected from the Microsoft form. You do not need to worry about filling out any fields in the 'Details' section. 

 

4. Upon 'Save', the 'Processing Status' will be set to 'New' and a notification will be sent to hcom.agreements@ohio.edu, notifying them of the request.

Search Salesforce to see if the Clinical Site Location already Exists

  1. Log in to Salesforce: https://ouhcom.my.salesforce.com/
  2. Make sure you are in the 'Preceptor' application via the waffle
  3. In the Search Bar at the top of the page - search for the name of your Clinical Site Location.

 

4. When the search results appear - you're looking for a Clinical Site Location record. You'll likely have to scroll down on the page bit to find that section. 

 

5. If your new Clinical Site Location does not exist, create a new one from your Affiliation Agreement. 

6. If your Clinical Site Location exists, but is inactive - please email hcom.agreements@ohio.edu, notifying them you'd like to reactivate it. Please include any pertinant information. 

Create a New Clinical Site Location from the Affiliation Agreement

1. Navigate to the Affiliation Agreement you just submitted and select the 'Clinical Sites' tab along the top of the record, then select 'New'. 

 

2. Enter data in the 'Intake' section of the page. It will be the same data points collected from the Microsoft form. You do not need to worry about filling out any fields in the 'Information' section. 

 

4. Upon 'Save', the 'Request Status' will be set to 'New' and a notification will be sent to hcom.agreements@ohio.edu, notifying them of the request.

5. Now your affiliation agreement and Clinical Site Location requests are linked together and complete.
