Flexplace Agreement Form

This form is used when administrators request a Flexplace arrangement per the guidelines in Policy 40.063. This form should reflect the agreed upon Flexplace arrangement between the employee and supervisor.  

To submit your form: 

1. Locate the form in the Attachments section. 

2. Complete all required fields. 

3. Once finished, click the Upload Form button and attach your completed document.
