Workplace Alternative Arrangement Agreement

Under Ohio University Policy 40.107, any time related employees are in a supervisory relationship, a formal, public agreement must identify the relationship, establish conflict of interest safeguards, and reassign supervisory duties. This form serves as the agreement and will be placed in employee personnel files for all impacted employees. A non-family member will be appointed to assume supervisory duties in place of the related supervisor.
