SSA-1945: Administrators, Classified Staff, Student Employees

Form SSA-1945, Statement Concerning Your Employment in a Job Not Covered by Social Security is completed by new administrators, classified staff, and student employees to acknowledge that as an Ohio University employee you will not contribute to Social Security.

If you do not know your EID number, you may leave it blank.

To submit your form:

1. Locate the form in the Attachments section.

2. Complete all required fields.

3. Once finished, click the Upload Form button and attach your completed document.
