Emergency Service Leave Request

This form is used by employees serving as volunteer firefighters, paramedics, EMT and First Responders to request emergency service leave. Employees are eligible for 40 hours of emergency service leave in a calendar year. 

To submit your form:

1. Locate the form in the Attachments section.

2. Complete all required fields.

3. Once finished, click the Upload Form button and attach your completed document.
