Creating a New NNP/DSME Patient in Salesforce

The Nurse Navigator Program and Diabetes Self-Management Education (DSME) Program use Salesforce to log patient information at the time of intake as well as additional visits and education sessions. The article describes to how to add a new patient to the database. 

Login to Salesforce

Salesforce can be accessed at the following link: https://ouhcom.my.salesforce.com/

  • Please select "Log In with OHIO Login" and use your OHIO SSO information to sign in.

Creating a New Patient

  1. From the homepage you will see a red square for "DI NNP Patients". You can click on that square to view your "Recently Viewed" or "All" patients. You can also search the "All" list to find a specific patient.

 

2. If your patient does not already exist, select the 'New' tab in the upper right corner of the page (see below screen shot). 

 

3. A pop-up will appear that says "New DI NNP Patient" and provides the options of 'cancel', 'Save & New', and 'Save'. Click 'Save'. 

 

4. A new page will load and you will want to enter in the below fields:

  • Intake Date
  • Intake Type (Navigator, DSME, Both)
    • The answer to this field changes what the necessary intake fields are for the patient, as they vary by type. 
  • First Name
  • Last Name
  • Date of Birth 

Click 'Save'. 

 

5. After saving, fill out the additional Intake Fields provided based on the intake type by selecting the pencil icon. 

6. Click 'Save'. After this step, you can log a NNP Visit or DSME Session. Please view the instructions for how to submit those in the 'Additional Resources' section below.
