Email Instructions:
Departments must submit taxable uniform information via email to payrolltax@ohio.edu.
Please ensure the email includes the following:
1. Preparer Information
Include the contact details of the person preparing the submission:
- Name
- Email Address
- Phone Number
2. Approval
The email must be sent by the CFAO or their designated delegate to confirm approval. Include the following information for the Approver:
3. Employee Uniform Details
For each employee who has received a taxable uniform, provide the following information:
- Employee ID Number
- Last Name
- First Name
- Middle Initial
- Fair Market Value
- Item Description
- Recipient’s Email
You may include this information in the body of the email or attach a spreadsheet with the required fields clearly labeled.
