Record of Taxable Uniform

CFAOs or their designated delegates are responsible for submitting the Record of Taxable Uniforms via email. For questions or assistance, please contact the Employment Tax Accountant at payrolltax@ohio.edu or (740) 597-1721.

Email Instructions: 

Departments must submit taxable uniform information via email to payrolltax@ohio.edu

Please ensure the email includes the following: 

1. Preparer Information 

Include the contact details of the person preparing the submission: 

  • Name 
  • Email Address 
  • Phone Number 

2. Approval 

The email must be sent by the CFAO or their designated delegate to confirm approval. Include the following information for the Approver: 

  • Approver’s Name 
  • Title 

3. Employee Uniform Details 

For each employee who has received a taxable uniform, provide the following information: 

  • Employee ID Number 
  • Last Name 
  • First Name 
  • Middle Initial  
  • Fair Market Value 
  • Item Description  
  • Recipient’s Email 

You may include this information in the body of the email or attach a spreadsheet with the required fields clearly labeled.
