Resignation/Separation Form

This form may be completed by a department representative, University Human Resources, or the resigning employee in the event of an employee’s resignation or separation from Ohio University. In lieu of the employee’s signature, a signed resignation letter may be attached. This form is not intended to be used for transfers, promotions, or other changes. 

To submit your form:

  1. Locate the form in the Attachments section.

  2. Complete all required fields.

  3. Once finished, click the Upload Form button and attach your completed document.
