How to Submit a Success Navigator Referral Form - Student Portal

Overview

All HCOM students can submit a referral form to a Student Success Navigator for themselves or on behalf of another student through their HCOM Student portals. Success Navigators are notified of these referrals upon submission. This article explains how to login to the student portal and submit a referral. 

Accessing the Student Portal and Logging In

The Student Portal can be accessed using the following link: https://ouhcom.my.site.com/students/. You will use your OHIO email and password to log in. 

Upon logging in, you'll land on a homepage similar to the one pictured below. From here select "Submit a Student Success Navigator Referral". 

Submitting a Referral

The page that loads will provide additional guidance on reasons to submit a referral. It's important to note that the referral form must NOT be used to report emergencies. Emergencies include students who are actively suicidal, homicidal, or in need of immediate assistance and follow-up. Please contact Campus safety (Athens: 740-593-1911) or your local police department (911) for emergencies and submit a referral afterward.

Fill out all required fields on the page and then click 'Submit'.
