Overview
Faculty, Staff, and Students all have the opportunity to submit referrals to the Office of Student Success on behalf of students who may need additional support. Once a form is submitted, an email notification is sent to the Office of Student Success to Address. This article explains how to process Success Navigator Referral forms once received.
How Referrals Are Submitted
Students - submit referrals through their HCOM Student Portals: https://ouhcom.my.site.com/students/.

Upon submission, a Salesforce Flow sends an email alert to necessary users, based on the student of concern's campus.
Accessing a Referral Form
Referral forms can be accessed in a few different ways:
- Via the link in the email notification
- Via the homepage (see below screenshot)

If accessing via the homepage, be sure to use the appropriate list view, depending on what you're working on. Click the small drop down arrow to see different options.

Processing A Referral Form
When the Success Navigator reviews the referral, there are a few steps that must be completed.
- Link the Student Contact Record
- To do this, begin typing the student's name in the lookup field. This is a very important step so that we can view a history of the student's referrals from the Contact details page.

- Populate the Success Navigator responsible for this referral
- Update the Status
- Create a related Success Navigator Interaction if desired
- In this lookup field, select 'New Success Navigator Interaction' to create a new one.
