HCOM Staff - How to Submit a Student Success Navigator Referral Form

Overview

All HCOM staff, preceptors, or friends of students can submit a referral form to a Student Success Navigator on behalf of another student through this form. Success Navigators are notified of these referrals upon submission. This article explains how to access the site and submit the form.

Submitting a Referral

The page that loads will provide additional guidance on reasons to submit a referral. It's important to note that the referral form must NOT be used to report emergencies. Emergencies include students who are actively suicidal, homicidal, or in need of immediate assistance and follow-up. Please contact Campus safety (Athens: 740-593-1911) or your local police department (911) for emergencies and submit a referral afterward.
