How to Submit a Success Navigator Referral Form - Faculty Portal

Overview

All HCOM Faculty can submit a referral form to a Student Success Navigator on behalf of a student through their HCOM Faculty portals. Success Navigators are notified of these referrals upon submission. This article explains how to login to the faculty portal and submit a referral. 

Accessing the Faculty Portal and Logging In

The Faculty Portal can be accessed using the following link:  https://ouhcom.my.site.com/faculty . You will use your OHIO email and password to login.

Upon logging in, you'll land on a homepage like the one pictured below. From here select "Success Navigator Referral". 

Submitting a Referral

The page that loads will provide additional guidance on reasons to submit a referral. It's important to note that the referral form must NOT be used to report emergencies. Emergencies include students who are actively suicidal, homicidal, or in need of immediate assistance and follow-up. Please contact Campus safety (Athens: 740-593-1911) or your local police department (911) for emergencies and submit a referral afterward.

Fill out all required fields on the page and then click 'Submit'.
