Overview
Volunteers for the Diabetes Cat Camp are required to submit an application and additional supplemental materials through the Cat Camp Portal via Salesforce. Existing HCOM portal users can use their same email addresses to create an account. New users may also create an account and register.
Accessing the Portal and Logging In
If the volunteer has applied to HCOM before as an applicant, student, CTAC standardized Patient, etc., they will login as an Existing User using the following link.
If the volunteer is new to HCOM entirely, they can use the New User link.
Submitting an Application
Upon logging in, you'll want to start a new application by click the 'Start a New Application' button. If you've applied before, you may also see existing applications on this page.
To be considered as a Cat Camp Volunteer, complete the following items:
- Application
- Volunteer Agreement and Release Form
- Program Staff Code of Conduct and Standards of Behavior Form
- Protecting Children Training
- Shine a Light Training
- A Background Check
- Photo Consent Form
On the page that loads, you will need to provide basic contact, demographic, and involvement information. You'll also notice that on the left hand said of the page, there are different pages that can be completed. The pages do not need to be completed in any specific order and when they are complete, the box will be filled.
When finished filling out the page, click 'Save and Continue' to be progressed to the next page. Several of the pages require you download a form, sign it, and upload it back to the portal.
First, download the document by clicking the hyperlink (yellow rectangle below).
Then complete the form, save to your device, and use the upload box to attach it to your application (blue rectangle).
Lastly, click 'Save and Continue' to save and proceed with the application (green rectangle below).
Submitting Your Application
When all pages and forms have been completed through the portal, you will receive a notification that all required materials and your application have successfully been completed and no further action is needed.
