Leave of Absence Request - How to Submit

Overview

A leave of absence request is a formal process for requesting a leave from the Heritage College PHWC curriculum for an extended period of time or a short period of time. A short term leave of absence is defined as a planned absence, lasting no longer than two weeks during preclinical education years. 

An extended leave of absence must be approved by the Committee on Student Progress. The committee will contact you to schedule a meeting to discuss your request and desired leave.  Following the meeting, you will receive follow up communication from Student Affairs with a CSP decision. 

A short term leave of absence is managed by the Office of Medical Education. You will receive follow up communication, regarding your request.

All Leave of Absence Requests are submitted through the HCOM Student Portal. 

It is important to understand that, per the regulations set by the Commission on Osteopathic College Accreditation (COCA), the accrediting body for the Heritage College, students must complete all requirements for the DO degree within six years of the date the student matriculated.

Accessing the Student Portal and Logging In

The Student Portal can be accessed using the following link: https://ouhcom.my.site.com/students/. You will use your OHIO email and password to log in. 

Upon logging in, you'll land on a homepage similar to the one pictured below. From here select "Leave of Absence Request". 

Submitting an Absence Request

The page that loads will provide detailed instructions regarding leave of absence requests, please be sure to read it thoroughly. To submit your first or a new leave of absence request, click the "Create New Leave of Absence Request" button. 

On the following page fill out all of the required fields as pictured below. 

 

You will also be required to acknowledge the COCA, Financial Aid, and Insurance Impact statements. Please thoroughly read them and check the box once you understand the impacts. Once all of the fields are completed, the "Submit" button will appear.
