Overview
Second year medical students are required to fill out the Clinical Education Information form in their HCOM Student Portals. Information provided on this form is used by the clinical education team to help schedule clinical rotations in year 3. This will be requested from the Clin Ed team, usually in January of your OMS II year.
Accessing the Student Portal and Logging In
The Student Portal can be accessed using the following link: https://ouhcom.my.site.com/students/. You will use your OHIO email and password to log in.
Upon logging in, you'll land on a homepage similar to the one pictured below. From here select "Clinical Education Information Record".
Completing the Form
The page that loads will be the form that needs to be submitted - similar to the one pictured below. Please be sure to fill out all required fields (notated by the red bar).
Authorizations
You will also need to sign 2 types of authorizations; A Records Release Authorization and a Registration Authorization.
Once all fields and authorizations are complete, click "Submit".
