OMS III and IV Didactic OMM Absence Requests - How to Submit

Overview

Clinical medical students, (OMS III's and IV's) can submit Didactic OMM absence requests through their HCOM Student Portals. 

Accessing the Student Portal and Logging In

The Student Portal can be accessed using the following link: https://ouhcom.my.site.com/students/. You will use your OHIO email and password to log in. 

Upon logging in, you'll land on a homepage similar to the one pictured below. From here select "Absence Request".

Submitting a Didactic OMM Absence Request

On the page that loads, you'll be able to see all of the absence requests you've submitted for your previous OMS years. To submit a new request, please click the "Create New Absence Request" Button. 

On the following page, fill out all of the required fields pictured below. For Absence Type, be sure to select 'Didactic OMM'. This form is used to request absences for one date only. Return to My Absence Request Records to create a new record for each additional date. For Didactic OMM absences and other course absences on the same date, please submit one absence request form for the Didactic OMM course and a second absence request form for the other course(s) that will be missed.

If you select “OMM Small Group Lab” for Clinical Education Event Type, you will have the option of requesting one of two options for how to make up the absence if excused: a make-up SOAP note or an alternate site. 

If you request make-up SOAP note, you will need to submit an assignment via the homework post in your eMedley course feed which corresponds to the session missed. 

If you request an alternate site,you will need to view the information sheet linked in the page. The information sheet lists the locations, dates, and times for all potential alternate sites in order to assist you in making an informed decision about which site is right for you. For example, if you are attending an audition rotation in Akron OH, you would want to select the Alternate Site which is closest to you (Cleveland) and then choose which of the four Cleveland Groups has the time slot which fits best into your schedule. Please note that not all sites accept alternates and approval for a specific site/group is subject to availability and final approval by the IoR. 

OMM Absence Requests for illness, audition rotations/interviews, professional requests and doctor’s appointments require supporting documentation before they can be submitted. 

To upload supporting documents make sure all items are saved in 1 file. 

  • In the File Upload box, select "Choose File" > Navigate to where the document is saved > Select It > Open
  • You'll see that the file has been added and you can remove it if necessary. 

If you do not have all of the necessary supporting documentation, You may receive an email requesting additional information. Failure to respond to the email in a timely manner may result in cancellation of your request. 

Once everything is complete, click the "Submit" button. Once the request is submitted you will not be able to make changes. If you need to make changes or corrections to a locked record, please send an email to Dr. Kelli Buckner at bucknerk@ohio.edu.  

Once you submit the form, you will receive a confirmation email of the request. The request will be sent to the OMM faculty member responsible for approving or denying the request. Once it's reviewed, you will be notified via email of the status. 

Possible OMM Absence Request Outcomes

Excused Absence: If your absence is excused, you will receive an email notification with additional information and instructions regarding a makeup assignment SOAP note or attendance at the alternate site you selected. 

Needs Additional Information: You will receive an email instructing you to provide additional information or documentation through your HCOM student portal. Once the new information is received, it will go back through the review process. 

Unexcused: You will receive an email notification explaining that your absence request is unexcused and the possible consequences if you do not attend the OMM session. 

View Absence Requests

After you submit your request, you will return to the Absence Requests list page where you can view all of your absence requests and their statuses, as well as, create a new request.
