Each February, OMS2 students submit a Clinical Education Information Form through their HCOM Student Portals. This article explains how the APAs can review their submissions through the Student APA Dashboard.

Instead of using  Microsoft Forms to request a new Affiliation Agreement, these requests will now be submitted in Salesforce (beginning September 2025). Once the requests are submitted, the Agreement and Compliance Manager will review and submit for approval. This article explains how to check if the Affiliation Agreement already exists and how to create a new one.
